The contact tool asks for your name, email, optional phone number, general role, service or employment interest and preferred language. Enter only your own contact information. Do not enter or email diagnoses, Medicaid numbers, medical records, Social Security numbers, financial details or other sensitive information. Job seekers should not send identity images or background-check documents.
How the contact tool works
Form entries are used in your browser to prepare an email draft. This website does not submit or store those entries in a website database. You must open your email app and send the message yourself, or copy it into an email. Your email provider processes the message under its own policies. Ordinary email is not a secure channel for sensitive health information.
How we use inquiries
When you send an email or call us, we use the information you provide to respond, discuss your general interest and explain appropriate next steps. Information in an email may remain in our email service and related communications. We do not use this website to sell personal information or build advertising profiles.
